The Australian Defence Force (ADF) is unable to provide a severe food allergy free environment (such as from peanuts) in relation to the consumption of food during cadet activities. Such a risk may be life threatening for people who suffer from a severe food allergy. It may be in the members' best interest not to participate in the proposed activity, or to provide, at their own expense, sufficient food to cover the duration of the activity. TAS will wherever possible, provide individuals with specially prepared rations if the School is informed of life-threatening allergies. Alternatively, students may elect to provide their own food for cadet activities. All information relating to allergies must be registered with the school sister at the Medical Centre. This information is then distributed to staff as appropriate.Student First Name *Student Surname *Date of Birth *Year of Entry *Year 8, 2024Year 9, 2024Year 10, 2024Year 11, 2024Parent/Guardian Name *Address *Parent/Guardian Email *Parent/Guardian phone/mobile *Parent/ Guardian *AgreeBy completing and submitting the TAS Cadet Unit Enrolment form, as the parent/guardian of the above student, you certify you are aware of your child's involvement and agree to him/her being enrolled in the TAS Cadet Unit for Years 8 - 10 and in Year 11 (where applicable). The student and I understand the obligations required for the Activities ProgramI confirm a full medical history has been provided to TAS for the student My child is responsible for the uniform and equipment loaned to them and that all equipment is returned to the TAS Cadet Unit at the end of their involvement in the Cadet program or be liable for its replacement cost.Submit
